Around here a standalone grind generally works out between $8 and $16 per inch of stump diameter, measured across the widest point at ground level. It costs less per stump when it follows a removal we are already on site for.
Because the machine still has to be trailered out, unloaded and set up whether the stump is twelve inches or thirty. That fixed cost is most of the price on a single small one.

No. Once the stump is ground the root system is dead and breaks down underground over several seasons. A handful of species will push shoots off those roots for a year or two, and the mower deals with that. It is not the tree coming back.
Not straight into fresh chips, which tie up nitrogen as they break down. Either dig the chips out and backfill with soil, or plant a few feet to one side of where the stump was.
